Referrals & Admission

We do not maintain a waiting list. Referrals are considered on a first come, first served basis, and anyone is free to request services at any time.

Admission

How the decision gets made

We admit individuals we believe we can genuinely support, and who choose to receive services from us. Both halves of that sentence matter.

  1. Reach out

    Call or email us. Families, guardians and case managers are all welcome to make the first contact.

  2. Send the referral

    A Person-Centered Support Plan should accompany each referral or request for services.

  3. Meet the Member Identified Team

    We meet with the team, acquire the applicable records, and report to the Case Management Agency as required.

  4. Consider the match

    Admission is based on the individual's own choice and preferences, their support needs, their ability to access the community, available resources, and their match to the site where an opening exists.

  5. The decision

    The decision to admit is made by the Director in collaboration with the Program Team. If we cannot support someone well, we will say so rather than take the placement.

What to send with a referral

We work directly with Case Management Agencies across our four catchment areas and understand the reporting that comes with a placement.

  • The Person-Centered Support Plan
  • Current support needs and level of support required
  • Any Individual Behavioral Services and Supports Plan already in place
  • Medical information, medications and health protocols
  • Guardianship or legal representative details, where applicable
  • Anything that helps us understand who this person actually is

Funding

IRSS is a Home and Community-Based Services waiver benefit. For most individuals, services are funded through Medicaid HCBS waivers and authorized through your Case Management Agency.

If you are unsure what your family member is eligible for, your Case Manager is the right first call. We are glad to talk through what we provide so you can have that conversation with better information.

First Eye Care Services follows the HCBS Settings Final Rule, which Colorado codifies at 10 CCR 2505-10 8.7001.B.

Being straight with you

How discharge works

We only discharge individuals who choose to leave, or who we genuinely feel we cannot support. That decision is made by the Director in collaboration with the Program Team, based on the same factors as admission. If it happens, we provide notice to the individual and their legal guardian or representative, follow our Dispute Resolution policy, meet with the Member Identified Team, and report to the Case Management Agency. Nobody is asked to leave without a process.

Frequently asked questions

Is there a waiting list?

No. First Eye Care Services does not maintain a waiting list. Referrals are considered on a first come, first served basis, and you are free to request services and supports at any time. Whether we can accept a placement depends on whether an opening exists that suits the person.

How many people live in a host home?

A maximum of three individuals. That limit is what makes the setting a home rather than a facility, and we do not stretch it.

What if my family member needs behavioral support?

We assist individuals who require behavioral support services. Where needed, an Individual Behavioral Services and Supports Plan is developed and implemented, with data tracked, progress evaluated, and the plan updated on a regular basis.

Who handles medications?

Medication administration support is provided according to each person's level of self-medication. People are also educated about their own medications and potential side effects, particularly controlled substances, because managing your own health is a skill worth building.

Will my family member be able to get out into the community?

Yes. We provide transportation support for non-medical, social and employment activities, and community integration is a core part of what we do rather than an occasional outing.

What happens to my family member's money?

They keep as much control over their personal finances as possible. We manage resources, track receipts, keep an active record of all personal items, and teach money management skills along the way.

Can we raise a complaint or dispute a decision?

Yes, and we would rather you did. First Eye Care Services maintains formal Dispute Resolution and Grievance Resolution policies, both person-centered and agency-specific, along with a Human Rights Committee. Ask us and we will walk you through the process.
